Policy for Canceled/Missed Appointments
VERY IMPORTANT: TO AVOID BEING CHARGED FOR CANCELED OR MISSED APPOINTMENTS
In order to provide all of our patients the best care possible in a timely fashion, it is essential that we keep missed appointments to a minimum. When appointments are canceled or missed without enough notice, we cannot offer that time to other patients who are eager to be seen. Unfortunately, the increasing frequency of canceled or missed appointments without enough notice has required that we implement the following policy.
Appointment Type Notice Required
Canceled/Missed Fee
- Office Visits/Consultation: 1 full business day - $ 35.00
- EMG/NCS Studies: 3 full business day - $150.00
- Injections and Surgeries: 5 full business day - $150.00
Clarification:
A "full business day" means 24 hours not counting weekends or nationally observed holidays.
For example, if your Office Visit appointment was at 3 pm on a Thursday, you would need to notify us no later than 3 pm on Wednesday to meet the 1 full business day requirement.
If your Office Visit was scheduled for 2 pm Monday, you would need to notify our office no later than 2 pm on Friday to meet the 1 full business day requirement.
